IN-HOME SUPPORT

Support in the comfort of your

own home.

Support at home works best when it fits naturally around your life - not the other way around. At Care & Venture, our in-home support is built around your individual needs, preferences and goals. We take the time to understand your routines, your household, and how you like things done.

This service brings together daily living and household support, personal and self-care, and overnight and 24-hour care. All delivered by consistent workers who know you well. Support is tailored to your NDIS plan goals and reviewed regularly to make sure it continues to meet your needs.

What's included:

 

  • Personal care (showering, grooming, dressing)

  • Meal and nutrition support

  • Daily living skills and household tasks

  • Medication prompting and health-related routines

  • Building independence in self-care over time

  • Overnight support (sleepover and active overnight shifts)

  • 24-hour support packages

  • Half-day and full-day shifts

  • Support with appointments, schedules and routines

Group Bonding Outdoors

This service covers both community access & participation: getting out and navigating everyday life and social support & connection: building friendships, reducing isolation, and finding your people through shared interests.

COMMUNITY & SOCIAL SUPPORT

Connection, participation, and belonging.

Getting out into your community and feeling genuinely connected to others is not a luxury. It is an important part of a good life. We support participants to engage with their communities in ways that are meaningful to them, at a pace that feels comfortable.
 

Support is provided by a worker who understands your communication style and makes participation feel natural, not forced. We match carefully so the experience feels genuine, not like a service being delivered.

What's included:

 

  • Attending community events, activities and outings

  • Social and recreational outings tailored to your interests

  • Building friendships and social confidence

  • Interest-based groups, clubs and hobbies

  • Appointments, errands and everyday community tasks

  • Navigating public transport and community spaces

  • Supported socialising in low-pressure, comfortable settings

  • Exploring new environments at your own pace

03 

TRANSPORT

Getting where you need to go should never be a barrier.

It is not just a ride... it is your support worker in the car with you, making the journey comfortable and turning getting somewhere into part of a good day. Our transport support is provided by people who already know you, understand your communication preferences, and know how to make travel feel straightforward rather than stressful.

Whether it is a medical appointment, a social outing, work, education or simply getting out, we plan transport that fits your schedule and supports your goals. Familiar faces, flexible scheduling and genuine support from door to door.

What's included:

 

  • Transport to medical and allied health appointments

  • Transport to social activities and community events

  • Support for work, education and training travel

  • Flexible scheduling with familiar, trusted drivers

  • Active support during the journey, not just the drop-off

  • Door-to-door assistance where needed

  • Journey planning and coordination support

  • Community access supported from a transport base

Transport with Care & Venture is not a separate service bolted on. It is delivered by the same support workers who already know you making every journey consistent, comfortable and connected to the rest of your support

Most providers receive a therapy report and move on. We read it, understand it, and build it into how we support you every single day. We welcome communication with allied health teams and provide honest, real-world feedback on how strategies are working in practice.

04 

ALLIED HEALTH SUPPORT

Bringing therapy goals to life in everyday routines.

Therapy time is valuable. Progress also happens in between: at home, in the community, and in the everyday moments that make up a person's life. We work directly alongside your allied health team to make sure the strategies and goals set in therapy are being practised, reinforced and built upon consistently every day.
 

Our support workers take the time to genuinely understand what your OT, speech pathologist or physiotherapist is working toward and they carry that understanding into every shift. Families and clinicians consistently tell us this makes a measurable difference to outcomes.

What's included:
 

  • Attending handovers and staying aligned with allied health recommendations

  • Providing real-world feedback to therapists and families

  • Embedding therapy goals naturally into daily routines

  • Active collaboration with OTs, speech pathologists and physiotherapists

  • Skill-building activities that support long-term independence

05 

RESPITE & CARER SUPPORT

Real rest for families and carers 

Caring for someone you love takes everything. We offer planned, flexible respite so that carers and families can genuinely rest, not just step away. The difference is in the relationship. We build the connection between participant and support worker before respite begins, so the handover feels natural and familiar rather than disruptive.
 

Carers can switch off knowing their person is with someone who genuinely knows them, understands their needs and enjoys spending time with them. That trust does not happen by accident, it is something we invest in deliberately.

Respite through Care & Venture is planned carefully around the participant's routines, preferences and support needs. We communicate openly with families throughout and adjust as needed so that respite remains a positive experience for everyone involved.

  • Community-based respite outings

  • Short-stay and extended respite packages

  • Regular scheduled respite for consistent, planned carer breaks

  • Short-notice respite where possible

  • Clear, open communication with families throughout every shift
